How to refer clients into the Community Care Hub
Better Health Together is building a Community Care Hub (CCH) to strengthen the connections between our region’s systems of care. To make this work, we’re growing a strong network of Community Based Organizations that can both refer people into care and connect them with the services and resources they need to thrive.
When you refer someone to the CCH, they’re connected with a Community-Based Care Coordinator who helps them navigate and access the resources they need. These coordinators often come from the same communities they serve, bringing deep understanding, trust, and cultural relevance to their work. There’s no time limit on how long a client can receive support from their care coordinator.
Referral Partner Sends Client Information
A client is referred into the CCH by a Referral Partner directly through our Client Management System or using our partner referral form.
Client is Assigned to the Right Fit Partner
Our team receives the referral and matches the client with a care coordinator based on their unique cultural and community needs.
Status Updates & Closing the Referral Loop
Referral Partners can opt in to receive reports from their referred clients on their status.
The value to your clients
Connect your clients to a dedicated community based care coordinator with no deadline and at no cost to them.
The Community Care Hub helps pool community organizations’ capacity through a network of care for those who would benefit from a relationship with a dedicated care coordinator. There is also no time restriction for how long a client can work with their care coordinator, and discharge is based on needs being met.

Membership in an Employee Assistance Program (EAP) for your staff
BHT has partnered with Ulliance to provide CBWs in our region the Life Advisor Employee Assistance Program (EAP) at no cost to them. BHT recognizes that CBWs are critical to care coordination, and we want to ensure that support services are available to you and your family.
Access to Core Trainings
BHT is currently working towards creating more access for the community based workforce to the trainings they need to serve community. These include HIPAA compliance, motivational interviewing, trauma informed care, insurance navigation, and more. Seats will be offered in these trainings as available for referral partners.

Learn about our Client Management System for Referrals
Our Client Management System helps us screen for all the needs of our clients, not just the urgent ones. Screening can sometimes seem long and tedious, but it can help identify more hidden needs that the client may not even realize they have until they are asked directly.
