BHT, with the help of three partnering providers, reviewed submissions from six Care Coordinating Agency (CCA) applicants who responded to our Pathways Hub Request for Proposals. While each organization has strong care coordination programs, we selected the two who we felt could immediately serve the criminal justice-involved clients of this Pathways Hub Pilot launching November 2018.

We are pleased to announce that we will offer contracts to Community-Minded Enterprises and Spokane Neighborhood Action Partners to provide direct care coordination to at least 200 clients through September 2020.

For more information on the Pathways Hub and pilot program, watch these brief videos on the basics of the Hub model, or view this recent webinar by our Director of Community Services, Jenny Slagle.
