Our Community Care Hub provides resources for community based organizations and their workforce to access the support and training they need to serve. But sometimes, we need to come together to share our best thinking about how to build capacity across the region.

Apply to the Request for Proposals by October 1, 2025
BHT intends to fund the care coordination work of 10-20 Community Based Organizations employing 12-30 Community Based Workers between February 2026 and January 2027. Proposals should be for care coordination services provided by a minimum of one to a maximum of five (1-5 FTE) Community Based Workers (CBW) and a recommended .3 Supervisor FTE per CBW.
We share our ongoing commitment to community partners by identifying and developing funding opportunities, but what about community needs that are out-of-the-box?
In 2023, the BHT board allocated funds for community events to elevate our community support and live out our mission and values. Better Health Together is committed to sponsoring events and community engagement activities that promote community wellness and elevate community voices.
We invest in organizations that align with our Mission & Vision—especially those that are by and for impacted communities, are current BHT partners, promote equity education and community wellness (beyond fundraising or clinical care), center equity in practice, and have smaller budgets with limited funding access.*
We limit sponsorship to one award per organization each year. A single request may include the costs of sponsoring multiple events and activities.
We award sponsorship grant amounts up to $5k.
We require that organizations must be a nonprofit with 501 (c)(3) designation (in rare instances, BHT may approve sponsorship with proof of IRS application for a 501c3 to ensure equitable access)
We know from experience that capacity building is challenging, and we are developing strategies to support these efforts with a focus on organizations providing community based care coordination.
In 2024, BHT offered voluntary Organizational Capacity Assessments for our current community based care coordination partners, Community Resiliency and Community Linkages partners (two of our funding opportunities from 2023), and our broader partner community to understand current strengths and areas for potential investment. The need for grant writing support emerged as a common, high-priority theme.
As a result, Better Health Together (BHT) is offering $5,000 grant-writing stipends for community based organizations applying for regional, state, or federal funding. To request funds, organizations must identify a specific grant opportunity that they want to pursue in 2025. This can include private or public funding (local, state, or federal).

The Better Health Together Behavioral Health Forum approved $45,000 of funding to offset lost billable hours. These dollars provide direct payments to member organizations to replace income lost when staff members are engaged in training that prevents them from billing for their time.
