SDoH Integration Learning Cohort Kickoff

As you prepare and plan for the year ahead, please keep the following in mind:

When: At our first meeting we will collectively decide on the best time and day during the first week of each month for recurring learning sessions. Each learning session will last 90 minutes–2 hours, with opportunities to connect after for further technical assistance.

How: We will hold all learning sessions remotely via Zoom.

Who to send: The project manager and/or staff within your organization who is championing this work. Ideally, the same participant(s) will attend each learning session. We recognize this is a significant time commitment, which is why we’ll be planning the “when” together at our February meeting.

Intent: We hope this Learning Cohort will support your team in strengthening your project and allow for shared learning. The five proposed topics to cover this year are: (1) Working with primary care and managed care organizations, (2) change management, (3) equity, (4) data outcomes and measurement, and (5) sustainability.

What to bring: We’re asking you to share the following elements of your project with the group:

  • Top two expected project outcomes

  • Key partnership(s) related to your project

  • Population this project serves
