Hi Collaborative Partners,

We are so excited to announce that we will be hosting an in-person Collaborative celebration Thursday, Dec. 16, in place of our regular monthly meeting.

There will not be a formal agenda, just an opportunity to connect with folks we haven't seen in awhile!

If you plan to attend, we ask that you provide proof of vaccination upon arrival and wear a mask unless dining.

If you do not plan to attend, we will see you all via Zoom again in January!

Please come prepared with your Proof of COVID-19 Vaccination, and wear a mask

• Full vaccination is valid two weeks after last recorded dose:

  • Single dose for Johnson & Johnson (Janssen) vaccine.

  • Two doses of Pfizer-BioNTech/Comirnaty or Moderna vaccine.

• Records from doctor's offices or pharmacies are acceptable forms of proof of vaccination.

Originals, copies, or photographs on a mobile device are acceptable.

• All attendees, regardless of vaccination status, are asked to wear masks except when dining.
