Introducing our 2026/27 Community Based Care Coordination Partners

BHT has been facilitating care coordination in the region since our founding. From our early days of insurance navigation and COVID trusted messenger work, to our Waiver 1.0 Pathways initiative, we have prioritized investing in and supporting the community based workforce and the organizations who provide access to care and vital services to so many in our region. 

This year, we will celebrate 14 years as an organization having invested almost $90 million into health delivery systems improvements and coordinated social care. We have been working alongside our statewide partners to develop ourselves as a Community Care Hub, connecting folks to the support and social care services they need through a trusted and trained community based workforce. 

In 2025, we released a request for proposals to establish a network of providers who would help us serve those experiencing the highest levels of health disparities. We worked hard to center our process around those organizations with the knowledge of and commitment to community-based care coordination with a community-based workforce.   

Our process was met with overwhelming interest, with 52 applicants ready to take on this challenge with us. Our Board Community Care Hub Stewardship Committee invested over 180 hours between 6 committee members to score all applications and choose finalists.

This process has also given us a chance to learn and grow as an organization. When we started building our RFP, we heard from community that RFP processes often have too short of timeline for smaller organizations to find a grant writer, figure out the details of the grant and write the response—especially alongside all the other hats they were wearing on a daily basis. Because of this feedback, we built our timeline to ensure everyone had access to all of the information well in advance.  

We also heard that often with RFPs it is difficult to find resources to help and answer questions. In response, we hosted 3 Bidders Webinars and 12 consecutive weeks of Office Hours to answer any questions. And to be transparent, we posted the full application, our scoring rubric, and all required contract documents in July, long before the application formally opened in September. We hoped this would give our community the chance to really work through their applications before the due date on October 1st 

We also understood that there might be some unavoidable conflicts of interest in this process as a deeply interconnected community. To this end, we took extra care to ensure that BHT Board members who were interested in applying were not privy to additional information that wasn’t available to all possible applicants.  Additionally, in November 2025, the Board adopted a revised comprehensive Conflict of Interest policy to hold itself accountable for transparency, follow best practices for nonprofit Board governance, and ensure that every Board member understands their legal duties as a Board member of BHT. 

We have learned a lot along the way and wanted to take an extra moment of gratitude for those who have spoken up on what we can improve. Without these voices, our mission to radically improve the health of the region could not be achieved.  We look forward to more conversation and putting these recommendations into action. We want to stand hand in hand with our community, applying our learnings into future processes, and continue to build on the great people and organization’s working to make our region healthier.
