Medicaid Transformation Demonstration Project Survey

Our ACH will have to decide this year which optional projects laid out in the Medicaid Transformation Toolkit our region will be tackling in order to earn incentive dollars from Medicaid Demonstration. With so much potential for impact from each project it is hard to choose, however we know we need to be focused to succeed.
